SARMs: A Medical Review



SARMs (Selective Androgen Receptor Modulators) are a new class of substances with different chemical structure (eg arylpropionamides, bicycloidantoin, tetrahydroquinoline, and quinoline analogs) and are specific androgen receptors (AR) ligands. The extreme attention aroused by the SARM preliminary studies is due to the fact that they have shown several advantages over androgenic anabolic steroids (AAS), such as tissue selectivity, the best pharmacokinetic properties, and the receptor specificity for androgen receptor, associated with the reduction of undesirable side effects due to the use and / or abuse of androgenic hormones.

SARMs have the property of having the same effects as estrogen on bone and blood vessels, but have an opposite effect on the breast and uterus. Thus they will be able to fight against osteoporosis, without increasing the risk of cancer of the breast and the uterus. On the other hand, logical consequence of the way they act, they have no effect on hot flashes and slightly increase the risk of phlebitis. Currently it is raloxifene that is available in the treatment of osteoporosis. It is taken daily and meals have no negative effect on its absorption. Its beneficial effect on bone is reflected in increasing bone mineral density and reducing the frequency of vertebral fractures .

On the other hand, the studies carried out did not make it possible to demonstrate a decrease of the frequency of fractures of the neck of the femur or the wrist. Its main drawback is the increased risk of phlebitis and pulmonary embolism. Raloxifene should be routinely combined with calcium and vitamin D intake.
